The Department of Health Care Services (DHCS) recently convened a meeting to address a compliance gap identified in its 2023 audit: missing or invalid Type 1 National Provider Identifiers (NPIs) and lapsed enrollment among practitioners who order, refer, or prescribe (ORP) services under Medi-Cal. DHCS used the session to walk providers through upcoming requirements, outline a phased rollout, and enlist partners in driving early outreach.
At the center of this effort is a federal requirement that any practitioner who orders, refers, or prescribes Medicaid services be enrolled with a valid Type 1 NPI. That covers ordering labs or durable medical equipment, referring patients to other practitioners or facilities, and prescribing medications or pharmacy items. DHCS is now moving to correct the audit findings tied to this gap, framing the effort as a way to strengthen program integrity, protect state and federal funding, and improve oversight. Enforcement will begin as early as February 2027 and roll out incrementally through Q2 2028, starting with low-volume prescribers and progressively expanding claim edits across pharmacy, medical, and behavioral health systems.
Once enforcement begins, members may face delays accessing medical, pharmacy, or behavioral health services if their ORP provider isn't properly enrolled, in some cases requiring them to switch providers. Billing providers, including pharmacies, risk claim denials when ORP information is missing, incorrect, or tied to a non-enrolled provider. Pharmacy denials can occur immediately at point of sale, while medical denials surface later through FFS and managed care plan processes — a timing difference that will shape how quickly practices feel the impact.
For oncology practices, the stakes are especially high. Oncologists sit at the center of nearly every ORP function DHCS is targeting — ordering labs and imaging, referring patients for specialized treatment, and prescribing complex, time-sensitive regimens. A single unenrolled prescriber could trigger denied prior authorizations or claims for chemotherapy, supportive care drugs, or diagnostic testing, directly disrupting active cancer treatment. With enforcement starting as early as February 2027 and expanding steadily through Q2 2028, oncology groups should treat enrollment verification as an immediate priority — delays in cancer care carry a cost few other specialties face.
DHCS has outlined mitigation steps including early outreach to unenrolled providers, coordination with managed care plans and licensure boards, expanded call center support, and enrollment application assistance. Processing currently targets 90 days across provider types, though volumes near enforcement dates may extend that window, making early application essential. Providers can verify their enrollment status using the list on the Open Data Portal, and DHCS strongly urges anyone unenrolled to submit their application immediately to avoid processing delays and future claim denials.
